We arelooking to expand our team by employing a New to General Practice Nurse to workalongside our practices and PCN staff. Our PCN staff currently includes GeneralPractitioners, clinical pharmacists, physicians associates, social prescribers,health and wellbeing coach, care coordinators, nurse associates and a paramedic.

The New to GeneralPractice Nurse undertakes the early detection, prevention, and management ofcardiovascular disease and other long-term conditions. This post-holder isexpected to take a lead role in managing specific long-term conditionsfollowing structured mentorship and preceptorship and work within amulti-disciplinary team, supporting integrated care and personalised approachesto patient management.
This role will takea lead in the following areas:
Dementia Reviews and Support
Mental Health Reviews and Care
Learning Disability Reviews and Support
Hypertension and Lipid Patient Care
Assist GP with Care Home care provisionincluding weekly ward rounds
The post-holderwill be an integral part of the general practice team, working in each practiceand across the PCN, as well as participating in the wider practice nursenetwork and multi-disciplinary team. They are to participate in formal trainingevents and to promote best practice in their area of expertise.
When a PCN employsor engages one or more New to General Practice Nurse under the Additional RolesReimbursement Scheme, the PCN must ensure that each has the keyresponsibilities in delivering health services, as outlined in Annex B (sectionB21) of the Network Contract Agreement DES Specification PCNRequirements and Entitlements 2025/26.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
