Job Summary

Working as part of the multi-disciplinary team with the North Uttlesford Neighbourhood Hub, the post holder will support patients through their mental health journey to ensure it is person-centered, streamlined and seamless, improving outcomes for patients and providing cost savings.

Key Responsibilities
  • Serve as an accessible point of contact for residents needing mental health services.
  • Maintain contact from referral through to service access.
  • Provide practical support: encouraging attendance, overcoming barriers, occasionally accompanying patients.
  • Identify and facilitate access to community resources.
  • Introduce self-help information and resources.
  • Advocate for patients not directed to primary or secondary care or those with poor experiences.
  • Manage a caseload, update records in System 1, alert colleagues to risk, and comply with Data Protection and other relevant policies.
  • Participate in MDT meetings, supervision, training and team meetings.
  • Support colleague development and community link-building.

Essential Criteria

  • Excellent communication skills, making people feel at ease.
  • Strong organisational and time‑management skills.
  • Positive, flexible, creative problem‑solving ability.
  • Accurate and concise record keeping.
  • Knowledge or experience of mental health issues (personal or professional).
  • Calm in emergencies, professional in stressful situations.
  • Self‑motivated, able to manage a varied workload.
  • Attention to detail.
  • IT proficiency (Word/Excel) and willingness to learn other systems.
  • Flexibility to work out of hours if required.
  • Teamworking, respectful of boundaries, contributions to decision‑making.
  • Willingness to engage in supervision, appraisal and training.
  • Passion for improving wellbeing of vulnerable people.

Desirable Criteria

  • Empathy and understanding of mental health needs.
  • Experience in a support capacity.
  • Knowledge of self-help materials and local support services.
  • Current DBS check.
  • Current driving licence, ability to travel in West Essex.
