About us

South East Oxford PCN is one of the collaborative groupings of GP practices. It serves a defined population within the South East area of Oxford City, aiming to provide more integrated, proactive and personalised healthcare.

Purpose and Structure: PCNs were introduced as part of the NHS Long Term Plan to address the growing complexity of healthcare needs, especially among ageing populations with multiple long-term conditions. South East Oxford PCN brings together general practices to work collaboratively with:

  • Healthcare disciplines to join the traditional GP workforce
  • Community health services
  • Mental health teams
  • Social care providers
  • Pharmacies
  • Voluntary organisations

This integrated approach helps streamline care pathways and ensures patients receive coordinated support across different services.

Key Objectives
  • Improved Access: Enhancing access to care
  • Proactive Care: Identifying and supporting patients with complex needs before crises occur.
  • Health Inequality Reduction: Targeting deprived areas and underserved groups to ensure equitable care.
  • Population Health Management: Using data to understand local health trends and plan services accordingly.
  • Workforce Development: Employing additional roles such as clinical pharmacists, social prescribers, and care coordinators.

South East Oxford PCN supports the development of Integrated Neighbourhood Teams (INTs), which aim to deliver care closer to home.
