Job description

North Sedgemoor PCN comprises the medical practices of Axbridge & Wedmore, Symphony North, Cheddar and East Brent. We are looking for a PCN Manager to provide management support to the Clinical Directors and PCNBoard, to deliver local strategic objectives. This manager will ideally have knowledge of Primary Care, be comfortableworking strategically, suggesting options forchange, and being prepared to push the boundaries of established thinking aswell as have sound operational knowledge and acumen for Organisational Management,Human Resource, Finance and Budgeting.

Main duties of the job

The PCNpractices have agreed to work together to deliver the NHS Long Term Plan andNetwork Contract Enhanced Service. The post holder will be expected to deliverlocal strategic objectives and to deliver the following priorities:

  • supportthe sustainability and resilience of GP Practices in North Sedgemoor PCN.
  • supportthe implementation and co-ordination of the North Sedgemoor PCN as per the NHSLong Term Plan.
  • maintain the sustainability of the workforce in view of the growing demand forservice.
  • support change management across the Primary Care Network.
  • To fulfilthe requirements of the Network Contract DES.
  • Act as first point of contact for Practice Managers,coordinate with PCN team leads and ensure high quality, patient centredservices.
About us

North Sedgemoor PCN has two strong clinical directors in post who have extensive experience in this role and GP Partnership. Many PCN staff are embedded in Practice, supported by respective Practice Managers. There are three salaried GPs with clear areas of responsibility and leadership and three team leads for ARRS roles.

Job responsibilities

Hours: 37.5 hours will need to work a Friday

Salary dependent on experience but guide £25.00 - £28.00/hour

Purposeof the role

The purpose of the role is to provide managementsupport to the Clinical Directors and PCN Board, to deliver local strategicobjectives. The manager will ideally have primary care experience, becomfortable looking strategically, exploring new ways of working, suggestingoptions for change, and being prepared to push the boundaries of establishedthinking as well as have sound operational knowledge and acumen forOrganisational Management, Human Resource, Finance and Budgeting

The PCN practices have agreed to work together todeliver the NHS Long Term Plan and Network Contract Enhanced Service. The postholder will be expected to deliver local strategic objectives and to deliverthe following priorities:

1. To support the sustainability and resilience ofGP Practices in North Sedgemoor PCN.

4. Supporting change management across the PrimaryCare Network.

5. To fulfil the requirements of the NetworkContract DES.

6. Act as first point of contact for Practice Managers,coordinate with PCN team leads and ensure high quality, patient centredservices.

Key Roles& Responsibilities:

To work closely with the ClinicalDirectors and PCN Board to provide operational support and guidance to enablethe Network to meet its agreed aims and objectives.

Monitor and evaluate performanceof the PCN against objectives; identify and manage change

Develop and maintain effectivecommunication both within the PCN and with relevant outside agencies.

Prepare and annually update thePCN Strategic Development Plan (and any other written information requested bythe PCN management committee), oversee the implementation of the aims andobjectives.

Attend relevant local meetings inagreement with the Clinical Director and line manager; to share views andliaise with member practices and other agencies and relevant parties.

Operational Management - human resource, financial management, organisational management of North Sedgemoor PCN.

Influencing, leading andsupporting the development of excellent relationships across the network toenable collaboration

Financial administration and reconciliationof financial funds and maintaining accurate financial records.

Preparing a PCN budget/forecastand share a regular financial update to keep members informed.

Administer and monitor pay andreimbursement for staff.

Contracts management with otherorganisations in the project.

Creation and submission of fundingbids.

Liaison with PCN Accountants.

HumanResources Management

Oversee the recruitment andretention of staff employed on PCN Network projects, as well as support earlyplanning of further recruitment.

Ensure that all PCN staff arelegally and gainfully employed. Monitor skill-mix and deployment of staff,aligned to direction given from the Clinical Director.

Ensure that PCN staff haveappropriate pre-employment and governance checks, have suitable induction, andare adequately trained to fulfil their role.

Ensure that there is effectiveappraisal and monitoring systems for PCN employees.

Support and mentor PCN staff, bothas individuals and as team members where appropriate. Implement effectivesystems for the resolution of disputes and grievances including thedisciplinary system.

Keep abreast of changes inemployment legislation.

Maintain up to date HRdocumentation (including job descriptions, employment contracts, DBS, MandatoryTraining, immunisation records and employment policies).

Maintain adequate staffing levelsto meet PCN/Network commitments.

Data andIT Management

Managing the collection of datafor continuous quality improvement.

Ensuring team members have accessto lT to enable them to successfully perform in their job roles. Develop andmanagement of any websites and social media to be utilised by the PCN.

Manage information governance toinclude development and implementation of information sharing agreements,completion of data privacy impact assessments and other matters around GDPR.

Liaise with the ICB/NHSE regardingsystems procurement, IT funding and national lT development programmes.

Marketingand PR Communications

Develop and update promotionalliterature (to include any social media and internet platforms) to promote thePCN to the healthcare community and public.

Develop and implement a publicrelations programme to promote North Sedgemoor PCN (to include attendance andpresentations at events hosted by the Network, Somerset ICB, NHSE etc.).Attendance at meetings to include operational meetings, executive teammeetings, steering group meetings and external meetings of value to theproject.

General

Keep a log of PCN projects,milestones and objectives tracking progress of these objectives ensuring clearcommunication to the Clinical Directors and Board of progress includingtimely and clearly highlighting any areas of risk.

Overseeing meeting schedules,minute taking, preparation (including Agendas) and distribution of paperswithin 7 days of Board meetings.

To include completion of agendasand minute taking for virtual meetings keeping a list of actions to beundertaken and followed up as appropriate.

Primarysupport to the Clinical Directors in their roles and responsibilities

Supporting the CDs to providestrategic and clinical leadership to the network, developing and implementingstrategic plans, leading and supporting quality improvement and performanceacross member practices (including professional leadership of the Quality andOutcomes Framework Quality Improvement activity across the network).

Influencing, leading, andsupporting the development of excellent relationships across the network toenable collaboration.

Supporting the CDs to providestrategic leadership for workforce development, through assessment of clinicalskill mix and development of network workforce strategy.

Supporting network implementationof agreed service changes and pathways, working closely with member practices,the wider PCN and the commissioner to develop, support and deliver localimprovement programmes aligned to national and local priorities.

Take a leadership role insupporting staff, public and stakeholder engagement.

Developing relationships andworking closely with other network Clinical Directors, clinical leaders ofother health and social care providers, local commissioners and Local MedicalCommittees.

Attending meetings as required onbehalf of the Clinical Director or the Network and feeding back to the PCNmembers.

Prepare and annually update theNetwork Strategic Development Plan (and any other written information requestedby the PCN Board, oversee the implementation of the aims and objectives andfeedback on progress at Network meetings.
