A leading healthcare provider in the United Kingdom is seeking a Healthcare Assistant (HCA) to support GP practices in delivering high-quality, patient-centered care. The HCA will work collaboratively with healthcare professionals to improve patient access and outcomes, perform clinical duties, and assist in managing long-term conditions. The Healthcare Assistant (HCA) will work across the Primary Care Network(PCN) to support GP practices in delivering high-quality, patient-centred careby working collaboratively with multidisciplinary teamsincluding GPs, paramedics,nurses, pharmacists, social prescribers, and other ARRS-funded rolesto improvepatient access and outcomes. The role involves supporting clinical teams,undertaking delegated clinical tasks, promoting health and wellbeing, andcontributing to the smooth running of practice and PCN services. Workingautonomously within the PCN Home Visiting teams, the HCA will carry outlong-term condition and dementia reviews for care home residents and houseboundpatients, ensuring personalised care plans remain up to date, while also performinggeneral HCA duties such as health checks, phlebotomy, chaperoning, and woundcare.
Performclinical observations including blood pressure, temperature, pulse, oxygensaturation, height, and weight.
Carry outvenepuncture/phlebotomy in line with local protocols.
PerformECG recordings and ensure accurate documentation.
ConductNHS Health Checks along with onward referral.
Conductphysical health checks for patients on the SMI register.
Carry outwound care, dressings, and simple procedures under supervision.
Undertakeurine testing, specimen collection, and preparation.
Administervaccines (if suitably trained and competence signed off).
Assistwith chronic disease reviews (e.g., diabetes, hypertension) under nurse/GPsupervision.
Supportthe management of long-term conditions by providing lifestyle advice.
Undertakehome visits for patients.
ConductLTC reviews following agreed PCN protocols.
Supportdementia reviews and liaise with relevant clinicians regarding any concerns.
Assist inmaintaining and updating personalised care plans for patients in care homesand those who are housebound.
Work aspart of the PCN Home Visiting Team to deliver proactive, preventative care.
Providelifestyle advice on smoking cessation, weight management, diet, and exercise.
Educate and signpost patients about preventive healthcare and self-management.
Accuratelyrecord patient information in clinical systems (SystmOne).

Coast and Country Primary Care Network comprises of four local surgeries that spans the Cornwall and Devon Border covering averagely 36,000 patients. The surgeries include Holsworthy Medical Centre, Stratton Medical Centre, Bradworthy Surgery and Neetside Surgery.
