Job summary

Tonbridge PCN is seeking a motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within primary care. You will work with Paramedics, Clinical Pharmacists, ANPs and Care Coordinators to deliver high-quality, patient-centred care in the community.

Key duties include CGAs, frailty care, housebound and care home visits, vaccinations, and nurse-led clinics. You will mentor junior staff, collaborate with GPs, and uphold NHS standards to improve patient outcomes.

Qualifications

• Degree in Nursing or Registered Nurse Degree.
• Registered with Nursing and Midwifery Council.
• More than one post-registration qualification and CPD, e.g., certificates, diplomas, or courses at level 6.

Responsibilities

• Deliver nurse-led clinics and housebound/care home visits.
• Provide vaccinations and, where trained, FeNO testing, ECGs, ear irrigation.
• Develop CGAs and coordinate care with GPs and multidisciplinary teams.
• Mentor and supervise junior/novice nurses and other staff across the PCN.
• Uphold NMC standards and safeguarding responsibilities.
