Modality Partnership Walsall Division has a full‑time fixed‑term vacancy for 1 year for a PCN Care Coordinator. The postholder will be a high‑performing employee who will work as part of a multi‑disciplinary team at Kingfisher Berkley Practice. Salary for this job is £25,000 per annum WTE dependent on experience.

This job is suitable for individuals who have a passion for working in health care, making a real difference providing seamless patient care, and who are committed to make real changes to the health care services provided within Primary Care to improve patient care.

Main duties of the job

The role is both the delivery of evidence‑based practice for patients presenting with a chronic condition, and the provision of preventative health care to the practice population. It involves demonstrating critical thinking and skills in clinical decision‑making in the management of patients.

You will be the key link to the people whose care you are supporting, operating as a go‑to person to ensure that their care is seamless, they are active decision makers in their own care, and that everyone involved is working together. Care coordinators provide extra time, capacity and expertise to support patients in preparing for or in following‑up clinical conversations they have with primary care professionals.
