Job summary

Hamilton Specialist Referrals is seeking a Patient Pathway Nurse to join our RVN team. You’ll be the central liaison from admission to discharge, delivering high-quality client communication and coordinating inpatient care within our wards team.

The successful candidate will be an experienced Registered Veterinary Nurse (RCVS) with strong communication skills and a commitment to evidence-based practice, working in a referral environment with advanced care options.

Qualifications

  • Registration as a Veterinary Nurse with the RCVS

Responsibilities

  • Provide a consistent and client-focused experience by keeping clients informed throughout their pet’s journey, addressing questions, providing guidance on care plans
  • Oversee patient admissions and discharges, and manage inpatient care coordination whilst working clinically as a key part of our wards team. This will include I/V catheter placement, blood sampling, TPR & BP assessment, sedation/recoveries and nurse consults
  • Act as a pivotal link between clients, clinicians, and the Clinical Administration team to maintain clear and effective communication, supporting the best possible patient outcomes
  • Assist with audits and quality assurance within the hospital

We are currently seeking a Patient Pathway Nurse to join our exceptional team of RVNs at Hamilton Specialist Referrals.

Hamilton Specialist Referrals was born of the belief that there is room for a new wave of veterinary excellence across the UK. There is a strong focus on providing a personal and reassuring client experience from first contact through to discharge. Excellent clinical care delivered in a friendly and supportive manner to referring vets, is paramount to the team. The hospital is operational 24 hours a day, 365 days a year and is able to offer the most advanced treatment options available for orthopaedic, neurology, soft tissue, and internal medicine cases. We have a dedicated Imaging suite, which includes a 16-slice CT scanner, 1.5 Tesla MRI, fluoroscopy, separate radiography and ultrasound room.

How You’ll Make a Difference

As a Patient Pathway Nurse, you’ll play a crucial role in delivering exceptional client service and seamless patient care coordination. From admission to discharge, you’ll be the central liaison, ensuring clear, compassionate communication with clients and working closely with clinicians and the Clinical Administration team. Clinically, you will be based predominantly within our Wards team, providing high standards of care to a range of complex referral cases.

Working Pattern
  • 23hrs/week
  • Working 07:30-19:30 Monday & Friday
  • Permanent position
  • No on-call or OOHs
