About The Role

We’re looking for an organised, patient-focused Patient Pathway Coordinator to join our friendly administration team at Practice Plus Group. In this vital role, you’ll be the first point of contact for patients and healthcare professionals — making sure every patient’s journey runs smoothly from referral to treatment.

You’ll be joining a collaborative and supportive team that plays a key role in ensuring our Outpatient clinics and Theatre lists are managed efficiently and filled to capacity. Every day brings new challenges, and you’ll have the satisfaction of knowing your work directly contributes to improving patient access to high‑quality care.

Working 37.5 hours a week. You will work 8 hour shifts between Monday - Friday 9am - 5pm or 10.00am - 6pm, we work on a rota system so shifts will vary week on week.

Any weekend shifts will be deemed unsociable hours and there will have an increase in the hourly rate should you work them.

What you'll be doing

As our Patient Pathway Coordinator, you’ll:

  • Deliver a professional and courteous service to patients, ensuring every interaction reflects our patient‑centred approach.
  • Manage calls from patients, GP surgeries, and hospital staff, providing clear and helpful advice about appointments and procedures.
  • Coordinate and book Outpatient clinics and Theatre lists, ensuring all slots are fully utilised and scheduling adheres to booking guidelines.
  • Process referrals, manage waiting lists, and ensure accurate and timely updates to patient records on our systems.
  • Prepare and distribute patient notes to the relevant teams, ensuring all documentation is complete and accurate.
  • Proactively identify and resolve issues that may affect clinic capacity or waiting times.
  • Work closely with clinical and administrative colleagues to ensure a seamless patient experience.

This is a varied, fast‑paced role where you’ll make a real impact on patient satisfaction and operational efficiency.

What we’ll look for in you

With your previous experience in an administrative or reception role, ideally within healthcare or a customer service environment, you will also have:

  • Excellent communication skills, with the confidence to handle a high volume of calls in a professional and reassuring manner
  • Strong computer and data entry skills, including experience with Microsoft Office and Patient Administration Systems (or equivalent)
  • The ability to stay calm under pressure, prioritise effectively, and work accurately to deadlines
  • A proactive, team‑oriented attitude with a commitment to delivering excellent service to patients
  • An understanding of confidentiality, data protection, and information governance
