Job summary

Allscreens Nationwide Ltd is seeking a Registered Nurse to lead safe, timely patient admissions and discharges, identify delays and coordinate with clinical teams to keep patients on track.

You will advocate for patients and families, chair multidisciplinary discharge meetings, ensure up-to-date plans, monitor progress, and maintain clear records in electronic systems, aligned with NICE guidelines.

Qualifications

  • Registered Nurse with active license.
  • Significant clinical experience in care coordination and discharge planning.
  • Strong understanding of discharge planning processes and related legislation/guidelines.
  • Excellent communication and interpersonal skills; advocate effectively for patients.
  • Ability to work autonomously and manage competing priorities in a fast-paced environment.
  • Proficient in using electronic health record systems for documentation and information sharing.

Responsibilities

  • Lead safe and timely patient admissions and discharges; identify and address discharge delays.
  • Advocate for patients, carers and relatives; facilitate sensitive discussions and MDT discharge planning meetings.
  • Develop and implement patient-centered admission/discharge plans with clinical staff; monitor and adjust plans to minimise delays.
  • Maintain concise, up-to-date records of admission/discharge plans and patient interactions in paper/electronic systems.
  • Provide guidance to ward staff on legislation, discharge planning best practices, and NICE guidelines.
  • Participate in data collection and audits related to admission/discharge and support discharge documentation.

Job description

Key Responsibilities
  • Patient Admissions and Discharge Take the lead in ensuring safe and timely admission and discharge of patients, actively identifying and addressing any delays in the discharge process.
  • Communication and Advocacy: Act as an advocate for patients, carers, and relatives to enhance their admission and discharge experience. Facilitate sensitive discussions regarding admission and discharge plans, addressing any disagreements and ensuring all parties are informed and aligned Multidisciplinary Collaboration Chair multidisciplinary admission and discharge meetings, ensuring that all relevant stakeholders are engaged in the discharge planning process.
  • Patient-Centred admission and discharge planning Collaborate with the clinical staff to develop and implement realistic and up-to-date admission and discharge plans that are centred around the individual needs of each patient. Monitor and amend plans as necessary to minimise potential delays.
  • Triage and Admissions Support Record Keeping Maintain concise and up-to-date records of admission and discharge plans and patient interactions using relevant, paper and electronic systems.
  • Education and Support Provide guidance and education to ward staff regarding current legislation, admission and discharge planning best practices, and adherence to NICE guidelines.
  • Data Collection and Audits Participate in the collection and presentation of data related to patient admission and discharge and support the development and maintenance of discharge documentation.
Qualifications and Experience
  • Registered Nurse.
  • Significant clinical experience.
  • Strong understanding of discharge planning processes and familiarity with relevant legislation and guidelines.
  • Excellent communication and interpersonal skills, with the ability to navigate sensitive conversations and advocate effectively for patients.
  • Demonstrated ability to work autonomously and manage competing priorities in a fast-paced environment.
  • Proficient in using electronic health record systems for documentation and information sharing.

Competence

You are responsible for limiting your actions to those that you feel competent to undertake. If you have any doubts about your competence during the course of your duties, you should immediately speak to your line manager / supervisor.

Risk Management

It is a standard element of the role and responsibility of all staff of the Hospice that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards. It is a requirement that you adhere to Nightingale House Hospice Policies, Procedures, Protocols and guidelines always.

Health and Safety Requirements of the Hospice

All employees of the Hospice have a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Employees are required to co-operate with management to enable the Hospice to meet its own legal duties including attendance at mandatory training updates and to report any hazardous situations or defective equipment.

Data Protection and Confidentiality

The post holder must treat all information, whether corporate, staff or patient information, in a discreet, secure and confidential manner in accordance with the provisions of the current data protection legislation and organisational policy. Any breach of such confidentiality is considered a serious disciplinary offence, which is liable to dismissal and / or prosecution under statutory legislation and the hospices disciplinary policy. This duty of confidence continues after the post holder leaves the organisation.

Records Management

As an employee of the hospice, the post holder is legally responsible for all records that they gather, create or use as part of their work within the organisation (including patient health, staff health or injury, financial, personal and administrative), whether paper based or on computer. The post holder should consult the IG Lead if they have any doubt as to the correct management of records with which they work.
