Patient Flow Coordinator

The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals. This role requires excellent communication with patients, families, and the multidisciplinary team to facilitate discharge processes and help patients get home as soon as they are ready.

Responsibilities
  • Plan and coordinate hospital discharges.
  • Communicate effectively with patients and families to involve them in preparing for discharge.
  • Work closely with adult social care and voluntary sector organisations to arrange appropriate support when patients leave hospital.
  • Maintain clear and accurate documentation.
  • Coordinate and contribute to daily board rounds and weekly multidisciplinary team meetings.
  • Adapt to a fast‑paced environment and solve problems as they arise.
