Job description

An exciting opportunity has arisen for a patient flow coordinator to join our fast paced intermediate care team unit in Hull.

The post holder will support the MDT in planning and arranging safe, effective discharges for patients. They will work under the guidance of registered practitioners with a diverse client group and will be joining an established multi-disciplinary team working at Rossmore Community rehabilitation bedded areas.

The role requires you to work with both Occupational Therapists and Physiotherapists as well as independently under the guidance of a Therapist.

The post involves working with people with physical problems within the community or hospital settings. You should be empathetic, have excellent communication and listening skills and the ability to support and motivate others to achieve their potential.

It would be helpful for you to have had experience of working under pressure. Experience of interprofessional working will benefit you in this role as well as a high level of IT skills. The successful candidate will be working in an exciting and forward thinking organisation that holds service and excellence at the heart of its values, and supports creativity and innovation in the workplace.

The organisation provides excellent CPD opportunities for staff, and your therapy team will be supporting you to develop your clinical skills. Where relevant, you may have the opportunity to work in other areas also.

This is a full time position of 37.5 hours working Monday to Friday.
