Job description
Role Overview

The role of the flow coordinator plays a key role in ensuring efficient patient flow by monitoring the patient's journey through the ED. This position serves as a liaison between clinical and administrative operations to help reduce delays and ensure that timely care is delivered for patients.

Responsibilities
  • Monitor and document patient movement through the ED using the electronic tracking system.
  • Identify and elevate delays in care.
  • Communicate in real time with the MDT to address flow barriers.
  • Support the ED nurse coordinator in managing patient flow and prioritising patient transfers to wards, including participation in huddles to identify patient flow issues.
  • Maintain accurate, real-time updates on patient status (discharge, admission, transfer).
  • Maintain momentum of discharge planning throughout the entire process, supporting and working in partnership with other members of the MDT (doctors, nurses, occupational therapists, physiotherapists and other hospital assessment and discharge team members).
  • Establish and maintain productive integrated working relationships with social service and community healthcare colleagues as required.
Qualifications
  • Demonstrates a willingness & capability to undertake NVQ level 3, Demonstrable experience in hospital setting working at NVQ level 2, Reputation for high standard of patient care.
