Job summary

Wyre Forest Health Partnership is seeking a motivated Practice Nurse to work 23 hours per week in a primary care setting. You will deliver nursing assessments, collaborate with GPs, and contribute to improving patient care standards within a multi-disciplinary team.

The role emphasises Chronic Disease Management, mentoring Health Care Assistants, and adherence to practice policies and infection control measures. Current UK registration is essential.

Qualifications

• Registered first level nurse.
• Clinical supervision training and experience.
• Mentor/teaching qualification desirable.

Responsibilities

• Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs.
• Strive to improve standards of care and be involved in identifying population needs; chronic disease management and mentoring of HCAs.
• Knowledge of GMS contract, including QOF/LES/DES, desirable.
