Woodbridge Practice in Stockton on Tees is seeking a skilled Practice Nurse to join our NHS primary care team. You will carry out Treatment Room duties, manage chronic diseases, deliver immunisations and contribute to health promotion for around 19,000 patients.

Qualifications

• Qualified Nurse with NHS primary care experience.
• Experience in long-term condition management (diabetes, COPD, asthma).

Responsibilities

• Carry out Treatment Room duties and clinical checks.
• Provide chronic disease care and follow care plans.
• Deliver immunisations, family planning and health promotion.
• Work as part of the primary healthcare team using SystmOne.
