A leading healthcare provider in Charlwood is seeking a part-time Physical Health Nurse to lead the coordination of healthcare needs for young patients aged 12 to 18. The successful candidate will facilitate healthcare plans, support a multidisciplinary team, and promote physical well-being. Applicants must hold NMC registration, have 2-3 years experience, and skills in NG feeding and phlebotomy. This role offers a salary of £31,000 along with various benefits including annual leave and free meals.
This is a part-time role as a Physical Health Nurse at Emerald Place Clinic, a general adolescent inpatient unit for the care and treatment of young people aged 12 to 18 with a primary diagnosis of mental illness and complex needs. The successful candidate will be responsible for facilitating physical healthcare plans, supporting staff, and promoting physical well-being among patients.
The Physical Health Nurse will lead and coordinate all aspects of the physical health needs of service users, while also supporting colleagues and promoting good teamwork with the multidisciplinary team. Key responsibilities include facilitating physical healthcare plans, ensuring routine tests are carried out, developing and reviewing care plans, educating staff, and promoting healthy living initiatives.

Emerald Place Clinic is a general adolescent inpatient unit for the care and treatment of young people aged 12 up to the age of 18. Young people who are admitted to the unit will have a primary diagnosis of mental illness across a wide range of disorders and complex needs.
These could include Mental Health conditions such as Mood Disorders and acute Psychosis. Additionally young people may have co-existing additional needs such as Neurodevelopmental Disorders, including mild Learning Disabilities or Autism. A young person could also be admitted if they have disordered eating.
