Responsibilities

• Provide face-to-face and telephone consultations as per timetable.
• Assess, diagnose, and manage acute, chronic, and undifferentiated conditions.
• Make safe, autonomous clinical decisions in line with guidelines.
• Prescribe safely in accordance with formulary and guidelines.
• Review laboratory results and clinical documents in a timely manner.
• Undertake relevant home visits where appropriate.
• Develop person-centred care plans and refer to secondary care when needed.
• Maintain accurate clinical records using EMIS and participate in governance and QA.
