Shap Medical Practice is seeking a Frailty Care Coordinator on a fixed-term basis until 31 March 2028, working 16 hours per week. The role involves proactive care coordination for frail, older patients, with duties across a multidisciplinary team to identify needs and arrange appropriate support.

The successful candidate will work closely with GPs, nurses and the wider team to maintain patient independence and home-based care where possible.

Qualifications

• Experience of working with the general public is essential.
• GCSE Maths & English at grade C or above is required.
• Desirable qualifications include NVQ Level 3 in Health and Social Care, Care Certificate and Dementia Awareness.

Responsabilités

• Care coordination, assessment and navigation across health, care and community services.
• Identify needs and coordinate support to help patients stay independent and at home.
