Job summary

A busy GP practice in Honiton is looking for an experienced diabetes nurse to lead the management of diabetes patients. This role includes delivering nurse-led clinics, collaborating with the multidisciplinary team, and supporting colleagues within the practice. The ideal candidate will be proactive, organized, and passionate about patient care, with relevant qualifications and experience in diabetes management. The position is for 19 hours a week, offering a dynamic and rewarding working environment.

Qualifications

• 2 years experience of diabetes management in primary care or a community or hospital setting.
• Experience of working autonomously.
• Experience of managing diabetes.

Responsibilities

• Deliver nurse led clinics to assess, diagnose, plan, implement and evaluate treatment/interventions for diabetes.
• Lead the planning and delivery of diabetes services.
• Undertake home visits for patients with diabetes as required.

Registered Nurse
