Job summary

The Forward Trust is seeking a Band 6 Community Substance Misuse Nurse to join our team in Thurrock. This part-time position (17.5 hours per week) delivers high-quality, person-centred care to people experiencing substance dependency.

You will work in a multidisciplinary team providing and supervising evidence-based interventions, triage and assessment, brief interventions, prescribed drug treatment, case management, crisis support, group sessions, and 1:1 work.

Qualifications

• Registered nurse with active NMC registration and substance misuse or community nursing experience.

Responsibilities

• Provide interventions and support for drug and alcohol users with complex needs.
• Develop, implement, and evaluate recovery-focused care plans.
• Conduct risk assessments, safeguard adults and children, and maintain accurate clinical records.
• Facilitate group and individual sessions using motivational interviewing and behaviour-change techniques.
• Collaborate with other professionals and agencies to ensure integrated care and maintain high service standards.
• Participate in ECGs, fibro scans, clinical audits, and health assessment clinics.
