Job summary

A healthcare provider in Chichester is looking for an experienced Paramedic Practitioner to deliver holistic care, assess, diagnose, and treat patients autonomously. Job description

The role will provide a specialist resource as aParamedic Practitioner for our patients working with clinicians, nursing teamand in collaboration with other members of the practice multidisciplinaryteams.

To work within our practice community, as anautonomous, accountable Paramedic Practitioner providing holistic care whilstassessing, diagnosing and providing treatment to patients.

To use competent and up to date clinical skills.

Main duties of the job

To assess, diagnose, treat, refer or direct to other services; our patients who attend surgery or request domicile visits relating to unscheduled minor conditions, injuries or acute illness.

Job responsibilities

Main Responsibilities

  • Minor illness assessment, diagnosis and treatment with access to seek advice, review and onward referral within clinical teams at The Croft Practice.
  • Patients requiring care and onward referral to secondary care to be discussed with available GP and this process to be reviewed ongoing with the Partners.
  • To see patients within clinics at The Croft Surgery as well as to provide care to patients on domicile visits and to develop skills within triage and telephone consultations.
  • To develop care plans for patients when required and to be involved with the continuity of care of our patients such as in end of life care, chronic disease management as developed and supervised with the Partners.
  • Maintain clinical knowledge and practice to known capabilities and competencies.
  • Ensure registration and qualifications with the Health and Care Professionals Council is maintained.
  • To undertake wound management within competencies and resources available.
  • Undertake the collection of pathological specimens with appropriate training and supervision including intravenous blood sampling, point of care testing and swab collection.
  • To maintain accurate, contemporaneous and concise electronic records within the practice, having received suitable training and supervision in the use of SystmOne Online software and read coding/template submission.
  • Involve in ongoing education within the practice and to support and provide training to staff members as identified.
  • Participate in annual appraisal with audit as identified and to develop and maintain Continuous Professional Development plans.
  • To work collaboratively with the Reception and Admin teams to ensure patient needs are met, supporting the team with their queries when required.
  • To ensure you have adequate and appropriate indemnity in place.

Prescribing and Medicines Management

  • To prescribe (as an independent/supplementary prescriber as applicable) for acute presentations and agreed long-term condition pathways, within own competence, clinical guidelines and practice protocols.
  • To undertake medication reviews relevant to paramedic-led presentations (e.g., acute infection treatment, analgesia, respiratory exacerbations), ensuring appropriate follow-up and safety netting.
  • To complete accurate prescribing documentation and coding within SystmOne, including indications, duration, review date, and patient advice.
  • To ensure safe prescribing practice including allergy checks, interactions, contraindications, renal/hepatic considerations, antimicrobial stewardship, and adherence to local formulary.
  • To discuss complex/high-risk prescribing decisions with the duty GP/supervising GP where required and document the outcome.
  • To contribute to medicines optimisation by signposting patients to self-care/OTC options where clinically appropriate and promoting shared decision-making.
Person Specification
Experience
  • Significant post-registration experience as a Paramedic, including autonomous clinical decision-making.
  • Experience in assessment, diagnosis and management of minor illness, acute presentations and long-term conditions within scope of practice.
  • Experience of working in a primary care, urgent care, or community setting.
  • Experience of managing patients requiring escalation, referral or admission, with appropriate GP/secondary care liaison.
  • Experience of undertaking home visits and/or remote consultations (telephone/triage).
Clinical Skills & Knowledge
  • Ability to assess, diagnose, treat and manage patients independently within competence and agreed protocols.
  • Safe and effective prescribing practice (where qualified), including medicines optimisation, antimicrobial stewardship and awareness of high-risk medications.
  • Strong clinical reasoning, risk assessment and safety-netting skills.
  • Ability to recognise clinical limits and seek senior input appropriately.
  • Knowledge of relevant national guidance (e.g. NICE), local pathways and primary care systems.
Qualifications
  • HCPC registered Paramedic with current registration maintained.
  • Relevant postgraduate qualification or equivalent experience in advanced/paramedic practitioner practice in primary care or urgent care.
  • Independent Prescribing qualification (V300) or working towards prescribing qualification with a clear plan and supervision in place.
  • Evidence of ongoing Continuing Professional Development (CPD).
