COCO PCN is seeking an experienced Paramedic to join our home visiting team across West Surrey and Sussex. The role involves autonomous clinical assessments, high-quality home visits and close collaboration with GPs and the multidisciplinary team to deliver the right care in the right place.
You will work across patients home visits and GP practices, developing care plans, coordinating investigations, and supporting patients and families with education and advice.
The Coco PCN Home Visiting Paramedic is an exciting opportunity for an experienced and motivated Paramedic to make a meaningful difference to patients across the local community.
Working across patients homes and GP practices, you will provide high-quality, compassionate and patient-centred care, using your clinical expertise to assess, diagnose, treat and support patients with a wide range of health needs. You will work closely with GPs and the wider multidisciplinary team to ensure patients receive the right care, in the right place and at the right time.
This role would suit someone who thrives on making a difference, is passionate about improving patient outcomes and enjoys working collaboratively with colleagues across primary, community and wider health and care services. You will have the opportunity to work autonomously while being part of a supportive team, contributing your ideas and clinical expertise to continually improve the service.
We are looking for someone with passion, drive and compassion, who is committed to supporting the community and delivering care that genuinely improves patients experiences and lives. If you enjoy building relationships, solving problems, working collaboratively and making a positive impact every day, this could be an excellent opportunity for you.
COCO PCN is a collaborative partnership between three GP practices in West Surrey and Sussex: Chertsey Health Centre, Ottershaw Surgery and Crouch Oak Family Practice. We are a forward-thinking, community-focused PCN committed to delivering joined-up, proactive and personalised care closer to home.
Our multidisciplinary team provides services including mental health support, paramedic home visiting, cancer care support and care-home coordination, working closely with local health and care partners to improve patient outcomes and experience.
We are passionate about developing and empowering our workforce. With two training practices, we actively promote learning, upskilling and knowledge-sharing across our teams. We aim to create a happy, supportive and knowledgeable workforce where colleagues feel valued, confident and empowered to contribute to improving our services and the care we provide.
