A healthcare provider in South Petherton seeks a Registered Paramedic/Registered Nurse to join their Rapid Response Team. The successful candidate will provide urgent care in patients' homes, manage caseloads, and work collaboratively within a multidisciplinary team. We have exciting opportunity for a Registered Paramedic/Registered Nurse to join our team who has a passion for providing urgent care in patients' home. You will be a dynamic individual who is able to work with unpredictable caseloads to assist in avoiding Hospital Admission's wherever possible.
We are seeking a proactive Registered Paramedic looking for a new challenge to join our Rapid Response Team which provides an Urgent Community Response to both support and provide unplanned short term care interventions to patients in their home environment.
The Paramedic/Registered Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required. Paramedic/Nurse will operate within agreed ways of working, following the relevant standards, policies and protocols.
The Band 5 Paramedic/Nurse is responsible for the holistic assessment of both complex and simple healthcare needs, the development, implementation, and evaluation of care, responding to clinical need and therapy need, and delegation of tasks where appropriate to other members of the team.
To independently assess, treat, review and discharge service users within the Rapid Response Service.
To work closely with the relatives, enablers, and carers within Somerset to holistically support the service user at home.
The post holder will be expected to work independently but have access to either a registered nurse, paramedic, or therapist for supervision and for development of specific treatment or discharge plans in appropriate situations.
Maintain a caseload of work with the expectation of adding visits to the clinical planner if on-going treatment is required.
The postholder is expected to step up where the Band 6 is absent, including caseload managing and chairing handovers.
The post holder will work as part of a collaborative team and join multi-disciplinary team meetings where necessary.
To support service users on either discharge from Hospital, or Admission avoidance pathways.
To undergo Urgent Community Response assessments with an unregistered or registered member of staff.
To upskill and supervise band 3 and 4 staff and students, ensuring their training and competencies are up to date.
