Responsibilities

• Understand palliative care approach and communicate sensitively with patients and families.
• Work collaboratively within the multidisciplinary team to deliver holistic care.
• Adhere to organisational policies, procedures, guidelines and protocols.
• Respect patient dignity, preferences and individual needs in care delivery.
• Observe and report changes in patient conditions promptly to a registered nurse.
• Maintain patient confidentiality at all times.
• Contribute to risk assessments and maintain a safe environment.
• Demonstrate understanding of the patient call system.
• Keep accurate, clear and timely records per standards.
• Work within your competence and seek guidance when needed.
• Recognise signs affecting wellbeing and report concerns.
• Document and report care using established procedures.
• Access resources to support patients, carers and colleagues.
• Seek advice or refer to MDT when appropriate.
• Respond to telephone enquiries professionally and relay messages accurately.
• Support volunteers within the hospice environment.
• Complete mandatory training and participate in development opportunities.
• Attend ward meetings and contribute to effective team communication.
• Engage in annual appraisal processes and personal development goals.
• Comply with Fire Regulations and Health and Safety at Work Act.
