Role Overview

Are you a compassionate and experienced nurse leader with a passion for palliative and anticipatory care? Do you thrive in supporting patients and their families through some of the most challenging times in their lives? If so, we would love to hear from you. We are seeking a dedicated Clinical Nurse Leader to champion high standards of palliative and anticipatory care within our community services. This role is ideal for someone who is enthusiastic about educating and empowering the workforce to deliver exceptional care at the end of life. End of life and palliative care is a key priority within the NHS, and we are committed to getting it right.

Qualifications
  • A wealth of experience in community nursing

  • A deep understanding of the complexities of palliative care
Responsibilities
  • You will work closely with district nursing and community therapy teams to ensure patients receive holistic, compassionate, and coordinated care.
  • Clinically this role involves providing anticipatory care for those with palliative conditions and additional support in community settings for palliative patients with psycho‑social complexity through autonomous clinical practice.
  • To be ultimately accountable for managing and planning care for patients with complex palliative and end‑of‑life needs in the community, escalating needs in a timely manner.
  • To work with primary care and partner agencies to ensure palliative patients are identified, anticipatory care is delivered and onward care is secured to reduce the need for secondary care intervention.
  • Play an active part in service improvements for Palliative and End‑of‑Life Care and management of people with complex needs in the Trust.
  • Contribute to Trust‑wide palliative and end‑of‑life pathway development, monitor pathway implementation and audit patient outcomes in relation to End‑of‑Life Care initiatives.
