We are seeking a dedicated Clinical Nurse Leader to champion high standards of palliative and anticipatory care within our community. This role is ideal for someone who is enthusiastic about educating and empowering the workforce to deliver exceptional care at the end of life.

You will work closely with district nursing and community therapy teams to ensure patients receive holistic, compassionate, and coordinated care.

Join us in making a meaningful difference in the lives of patients and families when they need it most.

Main duties of the job
  • Clinically this role involves providing anticipatory care for those with palliative conditions and additional support in community settings for palliative patients with psycho‑social complexity through autonomous clinical practice.
  • To be ultimately accountable for managing and planning care for patients with complex palliative and end of life needs in the community, escalating needs in a timely manner.
  • To work with primary care and partner agencies to ensure palliative patients are identified, anticipatory care is delivered and onward care is secured to reduce the need for secondary care intervention.
  • Play an active part in service improvements for Palliative and End of Life Care and management of people with complex needs in the Trust.
