Job summary

This post is based in the pharmacy, children's out-patient clinics and on the specialist ward at Leeds General Infirmary, although you may be required to work at other sites to support the service when necessary.

You will be part of a highly specialised clinical pharmacy service to paediatric liver and renal transplant recipients, primarily in an MDT out-patient setting, supporting medicines optimisation and patient centred care.

Qualifications

• Experience in paediatric pharmacy preferred.
• Excellent communication with parents/carers.
• Ability to work under pressure and within a MDT setting.

Responsibilities

• Work as part of a multidisciplinary team to support medicines optimisation.
• Co-develop and manage homecare provision of immunosuppression for paediatric transplant patients.
• Support medicines management for in-patients and develop the Specialist Clinical Pharmacy Technician role.
