Main duties of the job

We are seeking a compassionate and motivated Paediatric Diabetes Family Support Worker to join our Paediatric Diabetes Multidisciplinary Team, which includes Consultants, Paediatric Diabetes Nurse Specialists, Dietitians and Clinical Psychologists.

You will provide personalised support to children and young people with diabetes, and their families, both in hospital and community settings, promoting well‑being, independence and positive lifestyle choices. You will also help families navigate challenges related to health, education, housing and social care.

Reports to the Lead Paediatric Diabetes Nurse Specialist and Lead Clinical Psychologist.

Responsibilities
  • Provide personalised support to children and young people with diabetes, and their families, in hospital and community settings.
  • Promote well‑being, independence and positive lifestyle choices.
  • Support families with practical issues related to health, education, housing and social care.
  • Work flexibly across core hours and, when required, outside core hours during the week.
  • Travel between hospital, community hubs and patient homes as needed.
  • Collaborate with multidisciplinary team members and other health and social care staff to provide holistic and tailored support.
Qualifications, knowledge and experience
  • Educated to GCSE (or equivalent) standard, with a pass in English and Maths.
  • NVQ Level 2 or 3 in Care or equivalent.
  • Evidence of recent learning development in health.
  • Previous experience in a clinical environment and direct patient care.
  • Excellent communication skills, able to interact appropriately with patients, relatives, team members and other staff groups.
  • Flexibility to work across core hours during the week and on weekends as required.
  • Willingness to travel between hospital and community bases, including patient homes and community hubs.
  • Experience working within a team and a keen desire to learn new skills and develop professionally.
