We are seeking a compassionate and motivated Paediatric Diabetes Family Support Worker to join our Paediatric Diabetes Multidisciplinary Team, which includes Consultants, Paediatric Diabetes Nurse Specialists, Dietitians and Clinical Psychologists.
You will provide personalised support to children and young people with diabetes, and their families, both in hospital and community settings, promoting well‑being, independence and positive lifestyle choices. You will also help families navigate challenges related to health, education, housing and social care.
Reports to the Lead Paediatric Diabetes Nurse Specialist and Lead Clinical Psychologist.
