Post: OvernightCare Coordination Service Assistant
Pay: £16.38 this is the overnight enhanced rateother rates will vary but this can be discussed at interview
Hours: Part Time / Full Time Hoursavailable. This is a 24/7 service, so flexibility within these hours will be required, to deliver the service needs.

Overview of Role:
Physical Effort
Staff often remain seated, at one desk, for most of theworking day.
Mental Effort
Work pattern can be interrupted by the telephone or staffenquiries.
Workloadregularly re-assessed and prioritised to meet required deadlines.
The workingday can be varied, and, within minutes, this can include dealing with peoplewho have complex physical health needs, people who have called 999 because theybelieve they require an ambulance, a general enquiry regarding services, or apatient who cannot utilise mainstream primary healthcare due to historicbehaviours. Other days may be quite monotonous and involve processing CareCo-ordination Notes, to ensure vulnerable patients are able to receive the besthealthcare to meet their needs.
Emotional Effort
Requirementfor tact and diplomacy when dealing with queries from patients, relatives,staff and Medicalrepresentatives.
Supportingvulnerable patients and callers
Managingsituations that can often be emotional, given the patients health status.
Managingstressful situations i.e., workload and time pressures
Dealingwith conflict, over a variety of issues
Ability toremain calm when other people are panicking or distressed.
Information Systems:
Work with the IT support team, to ensure effectivemanagement of Information and Communications Technology within the service.
Information Governance:
Understand the importance of Information Governance and maintain the IG Toolkit on anannual basis.
Maintain accurate and comprehensive statisticalinformation, ensuring legal and local requirements are met.
Safeguarding
Staff areto report any safeguarding concerns to managers on site and follow usualprocesses if they become aware of any.Safeguarding is everyones responsibility.
Training and Support:
Identify own training needs and others and participatein relevant in-service education.
Ensure all mandatory training is undertaken andmaintained.
The post holder will be expected to attend anynecessary courses that provide relevant training.

This JobDescription will be periodically reviewed in the light of developing workrequirements. This is an evolving role and, therefore, these duties are notexhaustive. The role may change via discussion between the post-holder and linemanagers. The individual in post will be expected to contribute towards thatrevision.
