Durham County Council is seeking an Outreach Worker to join our Family Hub Teams, delivering group-based programmes and home visits to support children, young people and families in community settings. We are looking for enthusiastic and committed practitioners to join our Family Hub Teams supporting children and Families to deliver services in community settings.
The Outreach Worker will work with children, young people and families aged 0-19 (or 25 with special educational needs and disabilities), assessing their needs and delivering high-quality interventions predominately as a group basis, They will also help families navigate to appropriate services within and beyond the hub to ensure their needs are fully supported.
