  • Encourage and assist individuals in accessing treatment services and support from partner agencies.
  • Conduct initial screenings and brief assessments to identify risks and support needs.
  • Provide practical and emotional support tailored to individuals’ unique circumstances.
  • Assist clients in attending appointments and remaining engaged with necessary services.
  • Support individuals with housing, benefits, and referrals to health and social support resources.
  • Promote recovery, resilience, and positive lifestyle changes among service users.
  • Work collaboratively with housing providers, healthcare services, and other key agencies.
  • Maintain accurate and confidential records in compliance with organisational policies.
