Employer: Provide CIC (social enterprise). Location: South Essex, Basildon. Salary: £30,308 per annum (Yearly). Closing Date: 06/11/2025 23:59.

The role of Outreach Wellbeing Coach (OWC) offers an exciting opportunity to work for Provide on behalf of the Essex Wellbeing Service (EWS). As an Outreach Wellbeing Coach you will partner with other providers to improve the health and wellbeing within seldom heard from communities across Essex. Your focus will be on delivering one‑to‑one coaching, health promotion sessions and health checks, support with smoking cessation, weight management, and general well‑being of your clients. The role is in‑person, face‑to‑face with frequent travel, including evenings and weekends for outreach events such as Prides and festival‑type events. You will need a full UK driver’s licence and access to a vehicle.

Experience of community work is essential. The successful applicant will evidence relevant skills and experience but will receive comprehensive training, including a Health Coaching qualification and NHS training to deliver Health Checks.

Responsibilities
  • Work closely with target populations to build trust and engagement, identifying people who can benefit from support but are unlikely to access the service via usual referral routes.
  • Develop and deliver health promotion (HP) and behaviour change communication (BCC) interventions targeting hard‑to‑engage populations.
  • Deliver a tailored 12:1 coaching programme for hard‑to‑engage groups, improving health, wellbeing, and community connections.
  • Support self‑management, enhance client self‑worth and confidence, and provide smoking cessation, health checks and weight management support.
  • Encourage participants to engage with volunteering options in the local community as part of the EWS strengthening communities plan.
  • Maintain ongoing contact with clients, providing follow‑ups at set intervals to sustain outcomes and support community connections.
  • Facilitate outreach HP/BCC interventions (face‑to‑face or virtually) with target populations and partner agencies, including training and group sessions.
  • Maintain and use systems to monitor and record HP/BCC interventions, contribute data for reporting, and adapt interventions per service KPIs and contractual obligations.
  • Manage a caseload of individuals, determining contact points, frequency, and endpoint; attend multidisciplinary team (MDT) case load discussions if needed.
  • Build and maintain professional relationships with partner agencies, representing EWS and Provide in alignment with values and policies.
  • Engage in continuing professional development (CPD) to improve skills and maintain knowledge of health and wellbeing aspects.
  • Work independently to achieve personal objectives linked to the annual delivery plan; attend team meetings and support work requests from the EWS locality lead.
  • Manage workload, calendars, timesheets, annual leave records, and notify line manager of changes in personal circumstances.
  • Manage social media accounts to promote EWS, local events and initiatives, linking with other organisations.
