Job summary

Qualifications

• Registered Adult Nurse or AHP with current registration.
• BSc (Hons) in relevant subject or equivalent.
• Post registration qualification in Critical Care Nursing or equivalent Acute Care course.
• Teaching/Assessing/Mentoring qualification is desirable.
• ALS provider qualification.
• Level 7 or working towards physical assessment course.

Responsibilities

• Identify and manage deteriorating patients to prevent adverse outcomes.
• Contribute to development and review of deteriorating patient and resuscitation policies.
• Provide education and guidance to staff on early recognition and escalation of care.
• Deliver rapid clinical support during medical emergencies.

Job Overview

Frimley Health NHS Foundation Trust is seeking an experienced and motivated Outreach Practitioner to join our Rapid Response Team.

This is an exciting opportunity for a skilled clinician with substantial experience in the assessment and management of acutely deteriorating patients and medical emergencies. You will play a key role in supporting ward teams, promoting early recognition of patient deterioration, and helping to deliver timely interventions that improve patient outcomes.

The successful candidate will have experience contributing to the development, implementation, and review of policies relating to deteriorating patients and resuscitation. A strong commitment to patient safety, quality improvement, and the wider critical care agenda is essential.

You will work closely with multidisciplinary teams across Frimley Health, providing expert clinical support, education, and guidance to ensure the delivery of safe, effective, and high-quality care.

Main duties of the job

The Outreach Practitioner will be a key member of the Rapid Response Team, focusing on the early identification and management of deteriorating patients to prevent adverse outcomes, including cardiac arrest.

The role involves providing rapid clinical support and expert decision-making during medical emergencies, contributing to the development and implementation of deteriorating patient and resuscitation policies, and working collaboratively with multi-professional teams to enhance patient safety. The postholder will also deliver education and support to staff on early recognition and escalation of care.

Qualifications

Essential criteria

• Registered Adult Nurse/AHP
• BSc (Hons) in relevant subject or equivalent
