Provide high-quality, confidential, non-judgemental sexual health and contraceptive care across community and outreach settings.
Undertake asymptomatic screening, including supporting patients with self-taken tests for Chlamydia and Gonorrhoea, and performing venepuncture for Syphilis, Hepatitis and HIV screening.
Assess patients for HIV testing and perform rapid HIV testing where appropriate; offer Emergency Contraception and dispense medication under PGDs/PSDs.
Deliver contraception provision including Implanon insertion, maintaining competence through regular HUB clinical sessions.
Promote access to PrEP (Pre-Exposure Prophylaxis) across the community and provide initial information and signposting.
Conduct history taking, triage, risk assessment and provide tailored sexual health advice and behaviour-change support.
Work with vulnerable groups including sex workers, homeless clients, substance users, MSM, BME communities, women’s centres and asylum seekers.
Work flexibly across outreach clinics, partnership centres and community locations.
Develop and implement outreach action plans to support sexual health improvement for vulnerable communities.
Plan, coordinate and deliver targeted health promotion campaigns (local and national), ensuring alignment with service priorities.
Work with the Marketing Team to ensure outreach initiatives are visible and accessible in communities and online.
Use public health intelligence, local data and evidence reviews to identify need, shape outreach interventions and evaluate their impact.
Develop, coordinate and evaluate digital platforms and campaigns to enhance service awareness and uptake.
Engage and collaborate with local partners, health promotion agencies and community organisations to strengthen integrated working and improve service reach.
Contribute to the development of best practice models in outreach and share learning through local networks and national resources.
Support the implementation and evaluation of the Outreach Service strategy, contributing to audits, monitoring and service improvement activities.
Use public health and service data to analyse health needs and assess the effectiveness of outreach interventions.
Undertake small-scale research and service user experience assessments to inform decision-making.
Work in accordance with national and local policies, clinical governance frameworks and the NMC Code of Conduct.
Deputise for the Lead Outreach Nurse in their absence.
Maintain up-to-date clinical and professional knowledge, applying evidence-based practice within outreach delivery.
Support induction, teaching, supervision and continuing education of junior nurses, agency staff, students and volunteers.
Contribute to training needs analysis and development of teaching programmes relating to outreach and sexual health.
Participate in professional development activities and encourage skill development across the team.

What you’ll be doing
As an Outreach Nurse, you will:

Use local public health data to shape outreach plans and evaluate their impact.
Build strong partnerships with local organisations to enhance service reach and integrated working.
Contribute to service development, audit, research and ongoing quality improvement.
Support training, induction and development of students, volunteers and junior nursing staff.
What we’re looking for
You will be:
An RGN/RM with current NMC registration.
Experienced in sexual health nursing at Band 5 or above, ideally within nurse-led clinics.
Knowledgeable in contraception and STI management, with venepuncture and implant insertion skills.
Confident working independently in community settings with diverse and vulnerable groups.
Skilled in communication, triage, risk assessment and providing inclusive, non-judgemental support.
Competent in working under PGDs (and ideally progressing towards non-medical prescribing).
Organised, proactive and committed to continual professional development.
A team player who embraces flexibility, service innovation and a community-focused approach.
