Job summary

The Blackheath Hospital in London is seeking a Clinical Pharmacist on a 12 months fixed-term contract. You will perform medication reviews, support safe prescribing, and provide patient-focused advice within a busy hospital pharmacy team.

Required are an MPharm/BSc/BPharm, GPhC registration, and oncology experience with at least 2 years post-qualification experience; flexible shifts and multidisciplinary collaboration are essential.

Qualifications

• A degree in pharmacy MPharm, BSc or BPharm.
• GPhC registration as a Pharmacist.
• Oncology Pharmacy experience.
• Previous experience in a clinical hospital pharmacy role.

Responsibilities

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
• Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity
• Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance
