Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust in the Isle of Wight is seeking a care co-ordinator and care manager for the Older Persons Mental Health Service. The post involves assessing, treating and arranging care for older adults with mental health problems who meet access criteria, with strong partnership working with carers and community services.

The role requires live NMC registration as a mental health nurse (RN [MH]), or current registration as an occupational therapist

Qualifications

• Ability to work in partnership with carers, family members and community services
• Experience of assessing and managing older people with mental health needs
• Evidence of post-registration training in mental health care and related practices
• Knowledge of risk assessment and management principles

Job description

This is a care co-ordinator and care manager post in the Older Persons Mental Health Service on the Isle of Wight, run by Hampshire and Isle of Wight Healthcare NHS Foundation Trust. The post holder assesses, treats and arranges care for older people with mental health problems who meet the service's access criteria. The service is aligning its older persons pathways with all-age community mental health services, and the work centres on people with serious and enduring needs, with close partnership working alongside carers and community services. Applicants need registration as a mental health nurse, occupational therapist or social worker.

Qualifications

• Nurse Mentorship Qualification or relevant professional equivalent

Registration & licences

Experience

• Experience of assessing and managing people with serious and enduring mental health needs
• Evidence of ongoing development and post-registration training
• Post-registration training in evidence-based psychological treatments
• Training in risk assessment and management
• Evidence of undertaking audit work
