What you will do:

• Conduct essential assessments for minor equipment and adaptations such as rails and galvanised rails.
• Recommend simple adaptations by using your Trusted Assessor skills at Level 3 and Level 4.
• Collaborate closely with the Adult Social Care Team to ensure high-quality care and support for adults in need.
• Maintain and update accurate records of assessments and recommendations.
• Travel across different locations as required; therefore, a driving licence is essential.
