Job summary

Cygnet Health Care is recruiting an Occupational Therapy Assistant for Cygnet Afon Las in Corwen. The role supports the development and delivery of occupational therapy within a neurocognitive service and works under the supervision of a Highly Specialist OT.

Based at the new Corwen site opening in early 2027, the post focuses on assessments, interventions and promoting independence for men with complex needs in a multidisciplinary team.

Qualifications

• Enhanced DBS and references are required.
• Ability to work under clinical supervision of a Highly Specialist OT.
• Experience or interest in neurocognitive rehabilitation is desirable.

Your Role

Summary

The post holder will work at Cygnet Afon Las, supporting the development and delivery of occupational therapy provision as part of the interdisciplinary team. They will work under the direction and clinical supervision of the Highly Specialist Occupational Therapist.

Responsible for:

• Supporting the development and delivery of occupational therapy within the neurocognitive service.
• Working under the direction and clinical supervision of the Highly Specialist Occupational Therapist.
• Supporting occupational therapy assessments and outcome measurement.
• Delivering delegated individual and group-based interventions.
• Supporting individuals to develop and maintain daily living skills, meaningful routines, occupational participation and independence.
• Observing, recording and communicating progress to the Occupational Therapist and wider interdisciplinary team.

Cygnet Afon Las (Corwen, Denbighshire)

Cygnet Afon Las is a brand-new, 15-bed neurocognitive service for men, scheduled to open in early 2027. The service will provide specialist assessment, treatment, rehabilitation and longer-term support for individuals with complex and enduring neurocognitive needs who may be unable to access more traditional neurorehabilitation services or live safely and independently within the community.

Individuals may present with long-standing acquired brain injury, cognitive impairment, dementia or other neurodegenerative conditions, sometimes alongside complex physical health needs. Some may also have complex risk histories, including previous offending, and may be subject to Ministry of Justice restrictions.

Afon Las will provide a structured and therapeutic hospital environment in which individual needs and risks can be safely supported. Referrals may be received from a range of pathways, including community, neurosurgical, neurorehabilitation and custodial settings.

Through interdisciplinary working, the service will maximise person-centred outcomes, occupational participation, independence and quality of life, while keeping each individual at the heart of their care and rehabilitation.

As the Occupational Therapy Assistant,you will provide direct clinical interventions delegated by the Highly Specialist Occupational Therapist. You will contribute to the assessment process, deliver and review graded occupational interventions, and support individuals to work towards their agreed care and rehabilitation goals in collaboration with the interdisciplinary team.

Occupational Therapy Directorate

Under the leadership of our occupational therapy directors, we support over 165 Occupational Therapists. Working together as a large team we provide support to each other across the group, with the opportunity to be part of specialist interest groups & become involved in service development.

Every service line has a clinical lead occupational therapist to lead and develop evidence based practice and agreed occupational therapy pathway.

We ensure our occupational therapists have a voice through participating in service model development, utilising the expertise and support of our clinical lead occupational therapists.
