Occupational Therapist – Community Team (Aylesbury)

Location: Aylesbury, Buckinghamshire
Contract Type: Temporary / Full Time
Rate: £36 – £37 per hour
Working Pattern: Hybrid – mix of community visits, home working, and office-based meetings

An excellent opportunity has arisen for an experienced Occupational Therapist to join the Community Team within a local authority in Aylesbury.

About the Role:

You will play a key role in supporting adults in the community to live independently and safely within their own homes. This hybrid role offers flexibility, with most time spent working from home or out in the community carrying out visits and assessments. You will be expected to complete approximately five home visit assessments per week, ensuring that individual needs are identified and appropriate interventions and equipment are provided.

Key Responsibilities:
  • Undertake comprehensive assessments of service users in their own homes.
  • Recommend and arrange suitable adaptations and equipment to promote independence and safety.
  • Work collaboratively with multidisciplinary teams and partner agencies.
  • Contribute to timely and effective care planning in line with local authority policies and national legislation.
Requirements:
  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Previous experience within community-based or local authority settings.
  • Strong assessment and communication skills.
  • Ability to manage a varied caseload independently.
