To deliver high-quality occupational therapy interventions to adults with physical, cognitive, sensory, and mental health needs. You will focus on promoting functional independence, preventing deterioration, and enabling individuals to live well at home and within the community.
Complete functional assessments in line with the Care Act 2014 and local policy.
Prescribe, arrange, and review specialist equipment and home adaptations.
Provide expertise in areas such as moving and handling, seating, falls prevention, and environmental modifications.
Develop and review personalised intervention and support plans that enhance independence.
Work collaboratively with multi-agency partners including Adult Social Care, Housing, NHS services, and voluntary organisations.
Support timely hospital discharges and contribute to reablement and intermediate care pathways.
Uphold safeguarding responsibilities through risk assessment and management.
Maintain accurate electronic case records and reports to required standards.
Meet service targets and contribute to ongoing quality and performance improvement.
Supervise OT learners where appropriate and actively participate in CPD, supervision, and reflective practice.
Maintain HCPC registration and professional ethical standards at all times.
