What you will do:

  • Visit clients in their homes or other settings, such as hospitals, to provide hands-on support and guidance.
  • Assist clients in achieving their personal goals towards greater independence.
  • Regularly review and adapt reablement plans to ensure progress and address any changing needs.
  • Maintain accurate records of assessments and interventions.

Requirements:

We are looking for a qualified Occupational Therapist with at least two years of post-qualification experience. You must hold HCPC registration and possess a valid full UK driver’s license, as the role involves visiting clients in their own environments.
