Sanctuary Personnel Ltd is seeking a Locum Occupational Therapist to join the Reablement Team in Brent. You will assess needs, plan care, and support discharge from hospital through home-based interventions.

You will collaborate with Social Workers, Care Assessors and health professionals, visiting residents across Brent to deliver person-centred reablement and promote independence, with ongoing development opportunities and full-time preference.

Qualifications

• Degree in Occupational Therapy required.
• Recent UK experience as an Occupational Therapist.
• Experience within the NHS is preferred.
• HCPC registration is required.

Responsibilities

• Conduct thorough strengths-based assessments to develop personalised care plans.
• Recommend needed equipment and home adaptations to promote independence.
• Carry out home visits across Brent to implement reablement strategies.
• Collaborate with Social Workers, Care Assessors and health professionals for coordinated care.
• Maintain partnerships with hospitals and community organisations to ensure seamless service delivery.
• Be present in Brent for multiple sites, including homes and community locations.

What you will do:

• Maintain strong partnerships with hospitals and community organisations to facilitate seamless service delivery
• Be present in the borough for at least three days each week, working in diverse settings like customers’ homes, community locations, partner sites, and the Civic Centre

Requirements of the Occupational Therapist:

• Degree or equivalent in Occupational Therapy
• Recent UK experience as an Occupational Therapist
• Experience within the NHS
• Registered with the HCPC
