Job summary

Sanctuary Personnel is seeking an Occupational Therapist to join the First Contact Service Team in Sutton, London. This full-time ongoing role offers an hourly rate of up to £37.85, with a blend of office-based and remote working days to support flexible service delivery and community outcomes.

The role focuses on screen referrals, assessments, collaborative MDT work, equipment recommendations, and signposting to appropriate services, improving resident independence and safety.

Responsibilities

• Screen and prioritise complex referrals received through the Contact Centre to ensure immediate needs are met efficiently.
• Conduct person-centred, strength-based assessments to formulate effective support plans.
• Provide expert guidance to Enabling Independence Officers and Trusted Assessors to promote excellent service delivery.
• Work collaboratively within a multidisciplinary duty team to deliver comprehensive support to the community.
• Recommend and implement suitable equipment and adaptations, facilitating residents’ independence.
• Complete proportional assessments, effectively signposting residents to the appropriate services.
• Demonstrate safe and effective use of recommended equipment to clients and their families.
• Assess eligibility and manage processes for major adaptations, including structural changes and lifts.
• Present cases to Quality Assurance Panels as required to maintain high standards of client care.
• Liaise efficiently with Housing, Social Care, and partner agencies, ensuring coordinated support for residents.
• Assist in finding suitable rehousing solutions for clients when necessary.
