Job summary

South London and Maudsley NHS Foundation Trust is seeking a dedicated professional to provide care co-ordination and case management under the Care Programme Approach. The role focuses on engaging with service users with complex needs, including substance misuse, and delivering rehabilitation and integration with the community.

You will participate in MDT assessments, duties, and maintain accurate records in a safe, therapeutic environment while building rapport and maintaining clear boundaries

• Co-ordinate MDT activities, undertake assessments and duties, and maintain accurate records.

Job description

To provide care co-ordination and case management to a defined caseload of service users within theframework of the Care Programme Approach (CPA). The ongoing caseload will consist of service userswith complex care needs including substance misuse or addiction issues. The interventions that areprovided will focus on engagement, recovery, rehabilitation and integration with the community. To carry out MDT activities, such as assessments and duty responsibilities To be community focussed, agile, and adaptable. To be adept at building a rapport with clients, whilst maintaining firm boundaries. To co-ordinate a first-class service that includes assessment and monitoring of physical health, workingcollaboratively with the individual, and maintaining a safe and therapeutic working environment andaccurate records.
