What You Will Do:
Conduct holistic assessments to identify individual needs and create tailored care plans.
Recommend and arrange adaptations, including minor and major home modifications, to support independent living.
Advise on postural seating, moving and handling, and provide equipment to enhance daily functioning.
Offer rehousing recommendations for clients requiring more suitable accommodation.
Work collaboratively with a dynamic multidisciplinary team to deliver efficient, person-centred support.
Requirements:
HCPC registration as an Occupational Therapist.
Full UK driving licence, with the ability to travel to community visits.
