What You Will Do:

Provide a high-quality, person-centred Occupational Therapy service to adults with physical or functional limitations.

Conduct screening assessments and deliver clear, evidence-based recommendations to meet individual needs.

Work closely with service users, carers, and families to build resilience and independence.

Participate in safeguarding procedures, ensuring all service users receive safe and effective support.

Develop and review care and intervention plans, ensuring they align with each individual’s desired outcomes.

Promote and implement strengths-based approaches that empower individuals to achieve greater independence.

Collaborate with colleagues and partner agencies to coordinate care and achieve holistic, long-term outcomes.

About You:

Qualified Occupational Therapist with current HCPC registration.

Minimum of 2 years’ post-qualified experience, ideally within a community or social services setting.

Skilled in conducting functional assessments and developing tailored care plans.

Strong communication and interpersonal skills, with the ability to build rapport across diverse communities.

A valid UK driving licence and access to a vehicle are desirable, as the role may involve travel between sites.
