Job Overview

As a Band 6 Occupational Therapist within the Harlow Integrated Community Care Team (ICCT), you will play a key role in supporting the delivery of proactive, neighbourhood-based care that enables people to remain independent and well within their own homes and communities. Working as part of a multidisciplinary team, you will provide specialist occupational therapy assessment and intervention for individuals with complex physical health needs, focusing on preventing hospital admissions, facilitating timely discharge, and reducing health inequalities. You will work closely with local Primary Care Networks, community services, social care, voluntary sector partners, and neighbourhood teams to deliver coordinated, person-centred care plans that promote independence, wellbeing, and self-management. The role supports the wider neighbourhood agenda by fostering integrated working, early intervention, and population health approaches to ensure residents receive the right care, in the right.
