Job summary

Responsibilities

• Provide community assessments and goal-focused rehabilitation in Pathway 2 bedded care, patients' own homes and care homes.
• Develop specialist therapy assessment and treatment skills in a community setting
• Develop generic skills to assess a variety of complex patients
• Take part in therapy triage across Community Therapy, Bedded Care Therapy and Rapid Response
• Travel across the High Peak and surrounding area
• Work within a multi-disciplinary team
• Uphold the behaviours and values set out in the DCHS Way

Job description

Substantive post in the High Peak Integrated Community Team at Derbyshire Community Health Services NHS FT, covering Community Therapy, Bedded Care Therapy and Rapid Response. The work is goal-focused rehabilitation and specialist assessment for complex adults and older people in Pathway 2 bedded care, patients' homes and care homes, with triage involvement. The post involves daytime shifts including weekends and bank holidays, and driving across the High Peak, so a vehicle is essential. HCPC registration and Band 5 clinical experience in a physical setting are required.
