Job summary

Nottingham CityCare Partnership CIC is seeking an experienced Occupational Therapist to undertake autonomous assessments for individuals under the Integrated Respiratory and Home Oxygen Service. You will work within the MDT to develop care plans, support activities of daily living, and deliver education to staff and students.

Job description

Job Purpose To work as an autonomous practitioner, you will undertake comprehensive assessments to identify and meet the health needs for Individuals under the care of the Integrated Respiratory and Home Oxygen Service, as well as being part of the MDT in pulmonary rehabilitation. To carry out profession specific, and holistic, assessments, care plans and evaluation as required for patients with long term respiratory conditions, as well as additional comorbidities. This will include support with activities of daily living, energy conservation and anxiety management in relation to breathlessness, including palliative support and symptoms management. To be responsible for continuing the ongoing development of high standards of clinical practice with evidence-based focus. To work as required within IROS, liaising with patients, families and social services to assess individuals. Dimensions Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for client visits, meetings and training. To participate in pulmonary rehabilitation programmes with the MDT To carry out training of other members of staff as required. Support senior colleagues with the facilitation of student placements.

• 2. To advise the Team Manager in relation to clinical needs of the client group.
• 3. To take delegated responsibility for management duties as required.

• 5. To use information technology on a daily basis and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy.
• 6. To demonstrate flexibility, prioritise effectively and time manage self, supporting others to do the same. Be able to delegate appropriately.
• 7. To participate in the Lone Worker Tracking procedure as applies to the team.
• 8. To act as duty clinician for the process of referrals.
• 9. To undertake any other duties which may be reasonably required

Clinical Skills

• 1. To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. To ensure a high standard of clinical care for the clients.
• 2. To gain valid informed consent and to identify through assessment clients who lack capacity. To work within a legal framework with clients who lack the capacity to consent to treatment.
• 3. To undertake advanced comprehensive assessment and evaluation of adults presenting with a wide variety of respiratory, complex physical and mental health needs, utilising a high level of investigative and analytical skills.
• 4. To be responsible for carrying a client caseload and to provide highly specialist clinical reasoning in assessing, devising and evaluating individualised goals with clients.
• 5. To be responsible for coordinating the safe and timely discharge process the service as appropriate.
• 6. To be responsible for delivering patient care in a pulmonary rehabilitation setting including supporting with an exercise programme and delivering education sessions.
• 7. To risk assess, advise and where appropriate provide equipment including teaching clients / carers in its use.
• 8. To provide highly specialist advice and education to other members of the MDT around current best practice and evidence-based practice regarding the clinical management of respiratory clients. To drive quality, clinical service improvement and clinical excellence.
• 9. To be responsible for own continuing professional development keeping abreast of new trends and developments. Attend relevant conferences and training courses, supporting junior staff to do the same. Incorporate new developments into clinical practice.
• 10. To participate in the supervision framework as a supervisee and a supervisor, including the appraisal process. To be able to set own objectives and evaluate own learning and that of more junior staff.
• 11. To be a clinical educator for undergraduate students on University placements, supervising, educating and measuring their performance against the standards set by the degree level qualification.
• 12. To undertake CityCare's competencies.

Professional

• 1. To work to your Code of Professional Conduct.
• 2. To maintain a high standard of care to clients and carers including accurate and detailed documentation.
• 3. To be responsible for own Continuing Professional Development maintaining an up to date portfolio.
• 4. To facilitate and carry out 1:1 and group education and training to colleagues and junior staff to improve knowledge and practice.
• 5. To contribute to Clinical Governance agenda and apply Risk Management strategies.
• 6. To contribute to implementation of service development and change as appropriate.

To be actively involved in Clinical Governance, implement policies and proposing practice changes. To comply with organisations Lone Working Policy and local team guidelines at all times when working alone.

Planning and Organisation

To be responsible for own time and caseload management. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care. To use information technology on a daily basis to record information and statistics, to aid communication and produce reports. To ensure the Health and Safety of staff and clients at all times in accordance with Health and Safety legislation, Trust policy and procedures. To undertake any other duties which may be reasonably required.

Training

To comply with and complete statutory and mandatory training in line with Nottingham CityCare Partnership policy. To attend training events as appropriate to support your continuing professional development. To maintain a continuing professional development portfolio identifying learning objectives. To actively participate in your own supervision, annual performance review and personal development plan.
