Job summary

East of England Community Health & Care NHS Trust seeks a nutrition professional to assess nutritional needs, develop care plans, and deliver high-quality counselling in group and 1:1 settings. You will contribute to NICE-guided tiered support for children and adults while joining multidisciplinary teams to ensure quality discharge planning and ongoing management.

You will support service development, rigorous data collection, governance, and CPD, promoting public engagement and safeguarding

Qualifications

• Ability to direct and coordinate programmes of care autonomously and collaboratively.
• Ability to produce detailed, accurate nutrition care plans.
• Ability to translate complex nutritional information into patient-friendly format to support adherence to healthy eating.

Responsibilities

• Provide comprehensive assessment of nutritional needs and develop care plans for individuals and groups.
• Evaluate and support ongoing nutrition needs in line with NICE guidance across Tier 2 to children and young people.
• Attend multi-professional meetings to provide nutrition input, including discharge planning.
• Support service development, data collection, and reporting of outcomes to meet KPIs.
• Promote service to communities and engage target populations.
